What MMMM is

Mind-Muscles-Movement-Music is a research-informed educational framework for understanding the causes of muscular tension, pain and injury in performers, and for developing the capacity to prevent, resolve and move beyond them.

MMMM integrates research on performance-related-muscular-disorders (PRMDs), biomechanics, Surface Electromyography (SEMG), neuroscience, performance psychology, sport and exercise science, learning theory, and pedagogy. It treats fields usually studied apart as one connected system for assessing, understanding, and improving the physical and mental mechanisms in musical performance.  

At its core, MMMM translates complex scientific concepts into practical strategies for performance, recovery, and sustainable movement. Its purpose is to make visible and teachable something most training leaves implicit: how movement is perceived, learned, organised, and controlled, and how intention becomes muscular activity, movement, and ultimately music.

In short, MMMM combines how we learn, how we teach, how we move, how we play, and why we play.

The scale of the problem

Musculoskeletal tension, pain and injury among musicians, known as performance-related musculoskeletal disorders (PRMDs), are widespread and persistent. Studies suggest more than eighty percent of musicians experience them at some point in their training or career. They are not the exception. They are the norm.

Despite growing awareness and a wide range of interventions, including physiotherapy, stretching, somatic practices, strengthening programmes, medical treatment and education, many performers find only temporary relief, or none that lasts. For many professionals and younger players alike, pain becomes a chronic companion: under-reported, normalised, or quietly accepted as the unavoidable cost of high-level playing.

That these disorders persist despite decades of intervention points to a single conclusion. Current approaches do not fully address the underlying causes.

Where it came from

MMMM emerged from a personal recovery. Dr Heleen du Plessis, a cellist, spent several years in severe and persistent pain that nearly ended her career. She was physically fit, highly trained, and deeply engaged in awareness-based approaches, and she had tried most conventional treatments. Some gave temporary relief. Some made things worse. None explained the problem.

The turning point came when her condition was assessed using surface electromyography (SEMG). For the first time, patterns of muscle overactivity, inhibition and lost voluntary control became visible, patterns that neither she nor the clinicians trying to help her had been able to perceive through observation, sensation or conventional assessment.

What became clear was that many established approaches had failed not because they were wrong, but because they assumed a level of neuromuscular awareness and control that was simply not present. That limitation was not unique to her. It was widespread, even among highly trained performers. By comparing the interventions that failed with those that finally restored control, it became possible to understand why a given approach helps some performers and not others.

Through SEMG biofeedback, targeted exercises, movement training and a deeper grasp of biomechanics, coordinated muscular function was restored. She recovered fully. The result was not only freedom from pain, now sustained for twelve years, but marked improvements in playing, confidence and focus, and the resolution of severe performance anxiety. Being both the performer and the investigator made it possible to study not only the outcome, but how the change itself occurred: how perception develops, how control is learned, and how movement can be reorganised.

MMMM is shaped by Heleen’s experience as educator, performer, researcher, and recovered patient. What began as a personal recovery became a decade of research, teaching and practical application, and then a framework.

How it works

MMMM is based on a premise that reframes the whole problem: that many performance-related disorders arise not from physical deficits alone, but from limitations in how movement is perceived, learned, organised and controlled. There is often a gap between what is taught and what is learned, and between what a performer intends and what their body can actually do. Where movement is poorly understood or insufficiently taught, performers develop compensatory strategies that allow early progress but later become sources of tension, limitation and injury.

A connective model

MMMM can be understood as the mechanism through which mind, muscles and movement give rise to meaningful music making. It models, and allows the modification of, each link in the sequence by which intention becomes muscle activation, then movement, then musical outcome.

Traced this way, tension, pain or loss of control can be followed back to their point of origin and reorganised into more efficient, adaptable and sustainable patterns. The mind modifies movement, and movement, in turn, modifies the mind.

The missing link: mind and muscle

Many of the problems most often named, muscle imbalance, overactivity, poor posture, weakness, faulty movement, are usually not the primary cause of dysfunction. They are downstream effects of a deeper disruption in awareness, organisation and control, arising from an insufficiently developed connection between mind and muscle.

That connection is made through neural pathways, the network by which muscles are perceived, activated, coordinated, strengthened and controlled. Both clinical and teaching practice tend to assume this access is already intact. In reality it is often limited or disrupted, by injury, pain, stress, illness, lack of exercise, or habitual misuse. When the pathways are weak, instruction has nothing to land on. A performer can carry out an exercise with the wrong muscles, hold a posture that looks correct but is produced internally by the wrong ones, and reinforce the very patterns causing the trouble, all while making a genuine effort to do the right thing.

Before instruction can be acted upon, posture modified, movement reorganised or muscles conditioned, this connection has to be present and working.

The same neuromuscular system also supports concentration, awareness and regulation under psychological pressure. Developing and strengthening the mind-muscle connection is therefore the central goal of MMMM. It is the foundation on which awareness, perception, voluntary control, coordination, strength and efficient movement are all built. 

Beneath the surface

MMMM works at the level where problems originate, rather than treating symptoms in isolation or correcting posture from the outside. Similar symptoms can reflect very different underlying mechanisms, and meeting them with identical interventions risks addressing the surface rather than the cause. The aim is not simply to remove symptoms, but to reorganise the system producing them.

To do that, it combines movement education, targeted evidence-based exercises and SEMG biofeedback, developing mind-muscle connection, perceptual clarity, awareness, understanding and voluntary control together. Performers learn where their tension and pain come from, how muscles actually function, how to intentionally activate and regulate tension rather than simply being told to relax, how to distribute load and improve coordination, how to strengthen safely, and how to carry all of this into practice, performance and pressure. Training begins in simple, supported settings where accurate control can be built reliably, and is then transferred, step by step, into playing.

From invisible to visible

Surface electromyography (SEMG) is the tool that makes this possible. It is non-invasive: small electrodes on the skin detect the electrical activity of muscles as they contract and release, shown in real time as colourful waveforms on an iPad. It reveals detail that observation, sensation and awareness cannot reliably reach, letting a performer see the relationship between intention, activation, movement and sound, and identify inefficient patterns, refine perception, strengthen neural pathways and reorganise movement with precision rather than guesswork.

Well established in clinical medicine, rehabilitation and sport science, SEMG has until now been little used in music pedagogy. MMMM pioneers its application for musicians, and through it, awareness becomes something verified, not assumed.

The method in practice

At the centre of the method are evidence-based muscle-activation and self-resistance exercises. They are not add-ons or remedies. They are the primary means through which understanding of muscle function is developed, tested and refined. Where many exercise approaches treat improved control as a byproduct of movement, MMMM makes that control, the access, the accurate internal sense of a muscle, the voluntary command of it, the explicit aim.

The exercises begin in simple, stable, supported positions, so that accurate perception and control can be built without unnecessary strain on the neck and spine. They establish the functional starting position from which any movement should be performed, and a secure foundation from which movement can be progressively organised and carried into more complex actions in daily life and performance. Grounded in mind-muscle mechanics and shaped to the real demands of playing, they give a clear internal reference against which other movement, practice and treatment can be measured and integrated.

Their benefits are broad and interconnected.

  • They build the mind-muscle connection and the neural drive behind it, and develop a working understanding of the musculature. 
  • They increase control over activation, independence and coordination, and warm muscles through improved blood flow, making them more responsive and supple. 
  • They restore efficient patterns and correct imbalances, condition and strengthen while managing load on the spine and joints, and improve posture, alignment, balance and breathing. 
  • They build endurance and movement capacity, reduce tension, overactivity and spasm, and prepare the body for daily activity, practice and performance. 
  • They sharpen concentration under pressure, support confidence, lower anxiety and aid emotional regulation, and in doing so reduce the risk of injury and extend the working life of the performer.

The method also combines movement training with education. Performers learn the causes of tension, pain, injury, and inefficient movement, together with the principles of biomechanics, muscular function, and rehabilitation that underpin lasting change. Physical skills are developed alongside mental skills such as awareness, concentration, self-regulation, confidence, and the ability to monitor and adapt movement independently. The aim is not simply to perform exercises correctly, but to understand why they work, how they relate to performance, and how they can be applied beyond the training environment.

People are not just doing exercises; they are learning how to think about movement and become their own teacher.

The principles underneath it

A set of findings runs through the whole framework. Together they explain why it is built the way it is.

Movement is the foundation of performance. Technique is the ability to organise and execute movement so that musical ideas become sound. How we move directly shapes how we sound. When movement is not trained with the same care as the music, the pieces are practised but the system that plays them is left underdeveloped. 

Muscles are the mechanism of that movement: the skeleton cannot move or hold itself. Because the muscles responsible for a movement often sit well away from the part that appears to move, attention is easily directed to the wrong place, and a muscle can be put to work it was never meant to do.

Awareness, on its own, is not enough. It is widely emphasised, and it is valuable, but it gives no direct information about activation, timing, coordination or force. Efficient movement depends on awareness, perception, understanding and voluntary control together. Without control, awareness stays observational rather than transformative, and the gap between intention and execution, between what is perceived and what is actually happening in the muscle, remains.

Capacity determines performance, and capacity must be trained. Performers place athletic demands on the body, yet practising the instrument alone does not build the strength, endurance and coordination that sustainable playing requires. The same mechanisms that support movement also support concentration, confidence, resilience and regulation under pressure, which is why physical preparation and mental regulation are treated here as foundations of performance, not optional extras.

Much of this is decided early. The habits formed in early training shape long-term health, and instruction that is not informed by biomechanics, neuromuscular organisation and learning science can reinforce harmful patterns even when intentions are good. Inadequate movement training is not a minor pedagogical gap. Given its consequences, it is a serious risk to a performer’s health and longevity.

Exercises are not universally effective. Prescription tends to assume a prior level of awareness, control and strength, and the ability to recruit the right muscle. Where that assumption is wrong, the exercise loses its purpose: it is entirely possible to perform an exercise without ever activating the muscle it was meant to target. What matters is not the exercise itself, but what is done, when, how and why.

Flexibility, too, is often misunderstood. It is commonly treated as the product of stretching, but it depends on how muscles function rather than how far they can be passively lengthened. Mobility without strength and control can reduce stability and raise the risk of injury. The evidence for sustained static stretching as a standalone way to prevent injury is unconvincing, and it may in some cases weaken muscles and strain neural structures.

And pain is not a measure of progress. It is a protective signal, not an enemy to be pushed through. Harder is not better. Pain also rarely originates where it is felt: tension and strain are usually downstream of how movement and load are organised across the whole system, which is why effective help means understanding the broader pattern rather than treating the sore spot. Where a treatment or exercise is not helping, or is making things worse, there is usually a reason worth listening to.

From passive recipient to active agent

Most approaches to musicians’ tension, pain and injury focus on what is done to the performer. A clinician treats. A teacher corrects. A method is prescribed. The performer is positioned, above all, as the recipient of someone else’s expertise.

MMMM develops the opposite: the performer’s own capacity to regulate movement, reduce tension and take an active part in their recovery and development. It reframes performance-related tension, pain and injury not as an unfortunate side effect of serious artistry, but as a learnable, preventable and reversible outcome of how movement, perception and control are understood, or misunderstood, throughout training. Addressing those risks early and systematically is not a luxury for elite players or the already injured. It is a basic responsibility of performance education at every level.

Who it is for

MMMM is likely to be most valuable to anyone who has tried several approaches without lasting success, who understands what to do but cannot consistently do it, who feels the gap between intention and execution, or who wants to address the underlying causes of tension, pain and limitation rather than manage symptoms.

For performers, professional, pre-professional and amateur, it speaks to those who carry tension, pain, fatigue or injury, who cannot relax despite being told to, who feel held back by technical or physical barriers, who struggle with anxiety or loss of control under pressure, who have reached a plateau, or who simply want to know what instructions like relax, use weight, support or play freely actually mean in terms of muscle and movement.

For teachers and educators, it offers a clearer understanding of where tension and inefficient movement come from, how to respond when a student reports pain or cannot act on otherwise sound advice, how to give more accurate feedback, and how to select and sequence exercises according to a performer’s actual neuromuscular needs and stage of development.

For health and movement professionals, the interest lies in performer health and injury prevention, in SEMG biofeedback, and in genuinely interdisciplinary approaches to performance. And for parents, researchers, conductors, schools and tertiary programmes, it is a way to understand the real demands of performance, to build healthy habits from the start, to recognise risk before it becomes injury, and to strengthen the connections between performance, education, research and healthcare. The framework suits all ages, and reaches across performance, pedagogy, biomechanics, somatic practice, and programmes that integrate biofeedback, rehabilitation and technology-assisted learning.

Evidence and recognition

The framework has been presented in workshops, conference and festival presentations, and public lecture-demonstrations internationally since 2016. As co-author, a study of muscle activity and musculoskeletal pain in musicians with the use of SEMG was published in a peer-reviewed medical journal (Overton, du Plessis and Sole, 2018).

MMMM became the foundation of a new Performance, Health and Well-being course at the School of Performing Arts, University of Otago, offered at undergraduate and postgraduate level. The course was reviewed by specialists in medicine, sport science and psychology and strongly supported as an important addition to the curriculum. The exercise physiologist and SEMG expert Rowan Ellis described it as comprehensive, noting that it delves into the intricate and reciprocal relationship between mental, psychological and physical well-being. The University of Otago Academic Committee called it “a great innovation”.

What comes next

MMMM continues to develop through teaching, research and practice. A book, Mind-Muscles- Movement-Music, setting out the framework in full, is in preparation; it will present the underlying concepts, the supporting research and the practical applications in structured form.
